BILLING INFORMATION

Before your first appointment we will take your insurance information and determine what your benefit coverages are and discuss this with you.

Most sessions are 60 minutes and cost $130. If you have insurance your fee will be based on the contracted or out-of-network rate determined by your insurance company. Service coverage is based on your particular insurance plan and may be covered in part of fully by your insurance plan.

If your insurance is in-network you will be charged you copay or deductible due at that time. If your insurance is out-of-network you will be charged the full fee at the time of service. We will file your billing for you and reimburse you any amount your insurance company covers.

Payment in full is expected at the time of service. We accept Credit Cards, FSA Cards, HAS Cards, Check or Cash.

  • Blue Cross Blue Shield Michigan (BCBSM)
  • Blue Care Network (BCN)

CANCELLATION POLICY

If you are unable to attend an appointment we request that you provide at least 24 hours advance notice to our office. Since we are unable to use this time for another client please note that you will be billed for the entire cost of your scheduled appointment if more than 24 hours advance notice is not provided. We are unable to bill your FSA/HSA card or your insurance company for missed sessions.
